PHS Counselor Check-In Request

The PHS Counseling Department is here for you when you need us. We want to be available to you and connect with you when needed, even through remote means. Please complete this form if you would like to request a check-in with your School Counselor.

Upon submitting a request, your Counselor will work to follow-up with you in the next few days, but may not be able to respond immediately. In order to meet the needs of all students, check-ins may be kept to a minimal amount of time.

This form is NOT to be filled out in the event of an emergency. If you are in need of immediate assistance, or are in a crisis situation, please do one of the following:

  • Dial 9-1-1.

  • Call or text a 24-Hour Crisis Hotline.
